Wayne, for what it's worth, when I took PADI OW about two years ago, we did what the instructor called a "simulated" emergency swimming ascent. As I recall we never took the reg out of the mouth, and just exhaled very slowly as we made an ascent at the appropriate rate. I don't know exactly how that would compare to a "real" emergency swimming ascent, but it's probably typical of what a lot of instructors are doing these days.
