I have had my mask knocked loose, but never off, several times when another diver kicked me in the face, and twice when a large lobster bolting out of a hole into my face.
One time I had a strap brake during a dive, but I was easily able to hold my mask on my face as I surfaced and did my safety stop.
Cave and wreck divers in overhead environments and decompression dives sometimes carry a spare mask, but the additional mass and drag of a spare mask seems completely unwarranted for normal open water diving.
