Posted from the scuba-l listserv at Brown University;

The International Merchant Marine Registry of Belize (IMMARBE) has at last issued its final report on the capsizing of the M/V Wave Dancer on October 8, 2001 -- the tragedy that took the lives of eighteen members of the Richmond Dive Club and three of the boat's crew during Hurricane Iris.

The Report is available for downloading at:
www.immarbe.com/marine_casualty_report.html (Note that there are underscore characters between the words which appear to have spaces between them.) You will need Adobe Acrobat to read the file. The Report is 89 pages long including Attachments, and the PDF file is about 10 MB, so download times will be long for those without a high-speed Internet connection. If you are not interested in the details of the investigation, you should read Section 1 (Method of Investigation), Section 7 (Conclusions) and Section 8 (Recommendations.)

As expected, the Report blames the inexperienced Captain for almost everything, and points no fingers at the company that hired him and then failed to supervise his actions (and brings in over a million dollars per year in business to Belize!)
