The International Merchant Marine Registry of Belize (IMMARBE) has at last issued its final report on the capsizing of the M/V Wave Dancer on October 8, 2001 -- the tragedy that took the lives of eighteen members of the Richmond Dive Club and three of the boat's crew during Hurricane Iris.
The Report is available for downloading at:

The Report is 89 pages long including Attachments, and the PDF file is about 10 MB, so download times will be long for those without a high-speed Internet connection. If you are not interested in the details of the investigation, you should read Section 1 (Method of Investigation), Section 7 (Conclusions) and Section 8 (Recommendations.)
As expected, the Report blames the inexperienced Captain for almost everything, and points no fingers at the company that hired him and then failed to supervise his actions (and brings in over a million dollars per year in business to Belize!)
