I carry an Extractor made for bee stingers and the like along with some fine point SS tweezers in my dive kit.
I put a hot compress on the area and then use the extractor to suck out the spine.
However sometimes they don't come completely out. Sometimes they work their way to the surface and come out on their own. Other times the pieces seem to disappear. On a few occasions I have had calcification form around the spine piece. When that gets annoying enough I have the calcified lumps surgically removed.
Some of the worst pain I have ever felt was having a spine jammed all the way into my knee cap bone. It happened in the shallows at San Nicoals in heavy surge while hunting for lobster.
