A couple of questions; are you positive the bottle
was pressurized and then left on as it was put into
the water? Or, did they pressurize the reg and then
shut down the valve? One of the tell tale
indications could be if there was still gas in the
cylinder after it was retrieved, especially after
being unused.

One possibility; the tank was not pressurized
while in the water (w/valve open) and the reg
came unscrewed, or wasn't screwed in all the
way to begin with. This could be possible is the
2nd stage was not secured to the side of the
bottle and ther was even a slight swell - but
still remote. With a pressurized reg and
DIN connection, it would be very difficult for
the reg to just pop off. If the reg was pressurized
and it came off, that means the bottle was empty
and now contaminated with water.

Another possible reason - a low down dirty trick
by someone else on the boat. ;-).
