Unsafe "attitude" of "strokes"?

I understand now that a "stroke" is a diver with an unsafe attitude. I've always related unsafe to inferior instruction, lack of experience, cheap or ill-maintained gear, over-confidence and limited ability. But attitude? New one for me. Can anyone give a specific example of an unsafe attitude? Thanks.
