If this was the older fellow (63, I think) who was OOA, failed in the buddy-breathing attempt, and he descenced to the bottom (and died) and was recovered a couple of days later, my recollection is that he was not a student but a certified diver, though very recently certified and with very little or no experience post-class.
I don't mean to harp on whether he was a student or not, but given that theme of the post was instructor responsibility, I think there's a HUGE difference bewteen an instructor taking a student to Farnsworth (or any other advanced site) for a training dive and an already-certified diver making a bad judgement call as to wether or not he was capable of doing a specific dive.
