And...
You can order many different cuts and accessories from a custom maker. including spine pads, kevlar knees and elbows, thinner or thicker panels wherever you want them, zippers or no zippers, attached hoods, etc, and the list goes on.

The only thing OTR suits have over custom is price. If it fits great, if it doesn't then custom is the only way.
And stuffing someone in a size smaller doesn't count. Just because OTR suits are spongier also means that when they are stretched on the neoprene is already thinner where it is stretched over a larger body part reducing it's thermal effectiveness even further underwater.
