Matt, first off don't use surgical tubing. It is large and gets in the way. It also requires tie wraps what break and cut your face. Surgical tubing also wroughts.
What most of us use is small nylon wraped shock cord. To attach it you use what is called a fisherman knott. It is a half knott that pull against another half knott. Your mouth peice fits between the two knotts.
The position of the reg should be just below your chin. You will have to stretch the cord slightly to get it over your head. Optimal position allows you to grab the reg with your lips with out using your hands. If the necklace is to tight is pull the reg up under your chin and it takes away from the mobility of your head as well as becomes difficult to quickley put in your mouth.
